Are you unhappy with the service you received? Please give us a call so we can address your concerns. We are available Monday through Friday from 9am - 6pm EST at 1-844-402-4344. 

If you would like to formally file a complaint against us, you may do so in the following ways:

  • You may also register a complaint with our accrediting body, the Accreditation Commission for Healthcare (ACHC) using this form. You may submit the form via:

We will record, investigate, and do our best to resolve your concern. Within 14 days of receiving your complaint, we will provide you with a written response to the investigation and resolution plan.
